Dear Parent/Guardian,

Please complete this form to register your child(ren) for transportation for the 2023-2024 School Year, submitting one form for an entire family (up to five children) for each student who qualifies for transportation. If your family includes more than 5 transportation eligible students, please submit an additional form.

Please note that transportation is only provided if your residence qualifies, per Medway Public Schools Policy EEAA:

Kindergarten students except those living in close proximity to the school
Grade 1 through Grade 4 students who live more than 1 mile from their school
Grade 5 through Grade 12 students who live move than 1.5 miles from their school

If your residence falls below these thresholds (as defined as the shortest distance in Google maps), please do not fill out this form as your child does not qualify for transportation.
